Medical Insurance
We accept most major medical insurances, including Medicare and Medicaid. Most plans that require you to pick a Primary Care Physician will require that physician to issue a referral number to us. If you have questions, please call our office at 516-741-0055.
If your insurance requires a referral please bring it with you or reschedule your appointment. Only emergency care will be provided, a waiver will need to be signed.
You are responsible for your co-pay and deductable at the time of your visit, which are set by your health insurance. Due to rising office cost, we cannot afford to waive.
You may call your insurance and change your plan to one the office accepts. You can also look for a provider that participates with your insurance in the booklet sent to you by your health plan or you can call customer service which is located on the back of your insurance card.
